Club Sports Verifications | Cornell University Registrar

Club sports verifications are completed by the Office of the University Registrar to verify eligibility for students to participate in club sports. Completion of the verification may need information for other offices and will require ample notice. Time to completion will be approximately 7-10 business days.

The following must be submitted for each request.

1. Complete the OUR Verification Request.

2. Complete the organization verification form. Must contain the following information for each student:

Name
7 digit Cornell ID number
Signature (can be electronic but must be original signature, not in script text)

3. Submit completed forms in-person (245 Day Hall; M-Th, 9am-1pm, 2pm-4pm) or via Secure File Transfer to cab496@cornell.edu and cmk26@cornell.edu.

Forms will only be submitted to the certifying organization via standard postal mail, Cornell Box or Secure File Transfer. Team captains, coaches and other players will not be able to receive completed forms due to privacy regulations.
